The Clinical Care Classification (CCC) System, a national nursing standard, is a respected resource for documenting patient and nursing care plans for the electronic health record (EHR). It provides a set of standardised, coded patient care terminologies for EHR input in nursing diagnoses and outcomes, as well as in nursing interventions in both acute and ambulatory settings. This new edition of the Users Guide, written by one of the founders of the CCC System, has been modified into an abridged, easy-to-use version to help nurses learn quickly about the CCC System Version 2.5 and expedite their development of CCC-based plans of care. It clearly explains how to use the CCC System, including a description of the CCC model and examples of patient/nursing plans or care templates with their EHR screens and brief explanations.

About Virginia K. Saba

Virginia K. Saba, PhD, RN, FAAN, FACMI, President and CEO of SabaCare, Inc., has pioneered the integration of computer technology in the nursing profession for over 30 years. She spearheaded the nursing informatics movement; integrated computer technology in nursing; promoted distant learning technologies; and developed computer-based information systems for community health, including the Clinical Care Classification (CCC) System. She is an Informatics Consultant and has served as a faculty member in academia and as a Nurse Officer in the U.S. Public Health Service (PHS). Dr. Saba is a graduate of Skidmore College Baccalaureate Nursing Program and earned a Masters of Nursing Arts at Teachers College, Columbia University. She then received a Masters of Science in Computer Technology of Management and a Doctorate of Education in Educational Administration and Scientific & Technical Information Science at American University. She holds Honorary Doctorates from the University of Athens, Greece (2000) and Excelsior College in New York (2005). in 1997, Dr. Saba received the title of Distinguished Scholar from Georgetown Univrsity School of Nursing (GUSON). She also served as a full professor at the Uniformed Services University of the Health Sciences (USUHS), where she taught and integrated computer technology in the Graduate School of Nursing (GSN) Research Program. It was a prototype for an adult nurse practitioner (ANP) program using interactive video technology with participating VA medical centers. In the fall of 1997, students from eight VA medical centers across the nation participated in a Post-Masters ANP Certificate Program, culminating with its first virtual graduation in June 1999. Dr. Saba is a Past Chair of the Nursing Informatics Special Interest Group (NI-SIG) of the International Medical Informatics Association (IMIA) and was Chair of the Steering Committee of the ISO/TC215-WG3 Reference Terminology Model for Nursing. She served as a consultant and faculty member for the Excelsior College Distance Learning Informatics Program. Dr. Saba has authored several books, chapters, and articles, and has served on the original editorial board of Computers in Nursing Journal and other health care technology journals. She is a Fellow of the American Academy of Nursing (FAAN) and the American College of Medical Informatics (FACMI), and a member of the Sigma Theta Tau International Honor Society and the Academy of Medicine in Washington, DC.
